Side Effects of Systemic Treatments

Side effects associated with immunosuppressants (methotrexate, cyclosporine, hydroxyurea) can include liver or kidney damage, anemia, high blood pressure, dizziness, or an upset stomach.

People with weakened immune systems should avoid taking immunosuppressants. Cyclosporine should not be taken by people who have been exposed to high levels of ultraviolet light (UVB or PUVA).

Anyone who is or may become pregnant should not take methotrexate, cyclosporine, hydroxyurea, or retinoids. Men whose female partners may become pregnant should not take methotrexate. Women who are breast-feeding should not take cyclosporine.

Long-term side effects are not well known for biologic agents such as infliximab and etanercept because they are relatively new. However, there is concern that they may damage nerves or increase infections. For this reason, TNF blockers may not be recommended for people with multiple sclerosis or people who carry tuberculosis. One review of current studies on TNF blockers emphasized the need to consider possible side effects further.
